Convalescent or Long Term Care
Certified
home health aides can care for a patient for a few days
or a few weeks after an illness or on an ongoing basis when
there are chronic care needs. The service is individualized
to meet patients' and families' unique situations.
In all
cases, the agency nurse works with the patient and family
to identify the most appropriate service package. The aides
will provide necessary hands-on personal care, keep the
household safe and tidy, and provide for proper nutrition.
They also provide socialization and encouragement to enhance
recovery.
If preferred,
this service can be provided within an Alternate Family
Care Home as part of the North Jersey Coordinated Family
Care Program.
