Express
A certified
home health aide helps the patient get settled back at home
upon hospital discharge or after an outpatient procedure.
This short term assistance provides an aide who can bring
the patient home (but will not drive); help get comfortable;
shop; pick up medications or supplies; prepare a meal and
tidy up.
If longer
term assistance is needed, that can be arranged through
a call to the Intake Coordinator.
