North Jersey Coordinated Family Care
This
is a service where people in need of care or supervision
move into the home of a specially trained family care provider.
They join in the life of the host home, become part of the
household, and receive any personal care or assistance that
they need.
Each
participant receives room and board and household maintenance
as well as an individualized package of services.
The
care providers are all screened and trained. Supervision
and monitoring is provided by agency registered nurses.
Families
may choose this service for short term respite or convalescent
care. Or, the service may provide a long term living option
for a person who can no longer manage independently but
does not want to move into a nursing home.
